ProCare Medical Center
ProCare Medical Center is excited to be able to offer an alternative option for patients without healthcare
coverage. For a low monthly premium and a co-payment, due at the time of service, the patient can have access
to the physician whenever he/she is sick or needs to have a preventative care visit.
The agreement is between ProCare Medical Center, the physician, and the patient. It does not secure
healthcare benefits for the member to any facility other than ProCare Medical Center. It is important that you
understand this is not health insurance. It does not provide you with access to Hospitals, pharmacies or
specialty providers. All treatment will be rendered at ProCare Medical Center by Dr. Sang D. Tran. Please

**When blood is being drawn for labs, a one-time fee of $10.00 will apply for each visit.
